Information I collect
How I store information
Our working agreement and personal information will be stored in a loackable cabinet that I will only have access too. Session notes will also be hand written and stored in the lockable cabinet. This notes will contain if you have atteneded or not attended, the date, time and length of session. A very breif bullet points of what we have discussed.
My Laptop is password protected and smart phone has a unique passcode.
This personal information and notes will be stored for 5 years in accoradance to my insurance policy with Holistic insurance.
Confidentiality notice
Confidentiality may need to be breached if I feel ethically bound to seek assistance on a client’s behalf or on behalf of others. This would be the case if you have disclosed that a client is at risk of harm or causing harm to others, or others they refer to are at risk of harm. I will always try and have a discussion with a client unless circumstances prevent this. In rare cases such as this, I will, where appropriate, try to inform a client about what has been communicated and to whom. There are also some instances where I am bound by law to disclose information, those relating to drug trafficking, drug money laundering and acts of terrorism.
It is a professional requirement, for me to attend Monthly individual supervision to discuss and explore aspects of my practice. Clients will not identifiable during this process. My supervisor will also be a member of a counselling assosiation and will attend their own supervision.
Your rights
You have the right to ask to see information held by me about you. To do this please either ask me or submit a request by email. You also have the right to ask for information which you believe to be incorrect and rectified. I will endeavour to provide you with the information requested within 4 weeks.
We can discuss any questions or concerns you may have about the way that your information is being held.
